Where are you located & in what area/counties do you deliver to?
I am located in Matteson, IL. We will serve any patient in the state of IL.
Do you ship orders to members at home via USPS/UPS/FedEx & is there a charge for that?
UPS and no charge
Do you carry automatic blood pressure monitors? Scales? Canes? Walkers? Wheelchairs? Grab bars?
We specialize in incontinence, urological, first aid, wound care, and nutritional products. We recently added walkers and canes to our inventory.
How would you like to receive DME referrals? Is it best for us to give your info to the MD offices ordering the item or can we email/fax/call you with member and doctor’s info and the items they need?
Referrals can be faxed, emailed, or called in. It would be helpful and can speed up the process for the patient, if all information is provided to us at initial contact. However, if that information is not provided beforehand, after receiving the referral we will call the physician to obtain a certificate of medical necessity.
What is your fax number and email for orders to be sent?
Our Fax number is 708-248-5139 and our email is forcemile0612@gmail.com.
Is there a way to get a list from you of the exact items Aetna Better Health of IL Medicaid covers that you can fulfill?
Yes. We can send a list of all supplies we carry! Keep in mind that some things need prior authorization like nutritional supplements. Body wash and wipes are not covered by Medicaid. Although they’re not covered, we typically supply body wash and wipes as a courtesy with patient orders, when it’s in stock. We will verify all orders through Medicaid to make sure the items being requested are covered. If not, we will let you know.
Do you have or can you provide us with a template of what information you would like when we fax/email over a referral?
Referrals can be faxed, emailed, or called in. The information we need is a demographic or face sheet containing the patient’s name, birthday, RIN number, address, phone number, doctor’s name, phone number, email, fax number, address, and any incontinence, first aid, wound care, urological, canes or walkers, and/or nutritional items the patient needs. We also need an emergency contact person for the patient and/or a number for the patients POA, (Power Of Attorney).
